Wrap Plan Document Services

We will create a Wrap Plan Document and Wrap Summary Plan Description (SPD) for ERISA health and welfare benefits.

These will provide required ERISA reporting and disclosure language, documentation of benefit providers and eligibility requirements in addition to supporting a combined Form 5500 filing.

- Wrap Health and Welfare Plan Document;
- Wrap Health and Welfare Summary Plan Description (SPD);
- Full-time Employees Eligibility and Affordable Care Act Addendum, and
- Consent Action of the Directors' document for approval and sign-off.
Provide annual updates when health and welfare benefit insurance carriers or benefits change, or when new ERISA content requirements become effective.
